NEW California Workers' Compensation Claim Forms and Posting Notice

SAN DIEGO, CA, January 7, 2015 - Effective January 1st, 2016, the California Division of Workers’ Compensation (DWC) updated their benefit notices to align with current regulations.  Two commonly used forms, the Claim Form with Notice of Potential Eligibility (DWC1) and Employment Posting Notice (DWC7) have been revised. To be in compliance with the law, California employers must use the revised forms. (Linked below for your use.)

Claim Form with Notice of Potential Eligibility (DWC1) is to be provided to injured workers within 24 hours of notice of injury.

All California employers are required to display the Employment Posting Notice (DWC7) or face potential penalty fines of up to $7,000.

Please replace your existing DWC1 and DWC 7 forms as soon as possible. 

Further information will be forthcoming from your carrier as they begin to update their forms.
